Documents
Here is the library of all documents that are frequently requested or needed. All documents are Word document forms unless otherwise mentioned. You should print and email your filled forms as PDF (preferred), Word, or WordPerfect documents.
Students with access to the WebCT account should upload their documents there.
Application year 2009 (entering class 2010)
The following items must be submitted for an institutional composite/committee evaluation.
- Welcome Week survey (open until May 1)
- Subscription to Listserv
- Pre-Application (due by February 1) and subsequent MSAC interviews (preferably by April 1)
- Applicants for Dental School and Early Selection must aspire to complete the pre-application and both interviews early!
- Composite Letter Request and Checklist to formally open your application file to receive your letters of recommendation (due by February 1).
- Photograph (digitally uploaded to Blackboard).
- Recommendation Request and Supplemental Evaluation Forms
Print for each reference. GMU faculty may request access to an online supplemental evaluation form.
- Reference letters are accepted for applicants between January 1 and June 1 for early transmission. The latest date for letters of recommendation is August 1.
